Register as a New Patient

To register as a patient with us, we will need you to complete some registration documents. You can do this online or collect these directly from any of our sites or download them below.

Please check your answers carefully and ensure you have completed all the questions. Missing information or incorrect information slows down the registration process as our staff will need to contact you to gain the correct details.

How to Register as a New Patient Online

Complete the online form to register to become a patient at Charnwood Community Medical Group.

The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.

You will need to fill in all of the details to register at our surgery. You will also need to provide a copy of your ID and proof of address. For example, valid ID could include a photograph of your passport or driving licence. This is to check that you live in the local catchment area.

We may also ask you for details of your previous GP surgery and basic health and medical information.

Although you can choose a site to register with, as Charnwood Community Medical Group is one practice you may be asked to attend one of the other sites.

How to Register as a New Patient with Paper Forms

All registrations must include both forms below.

  • Form 1 - NHS PRF1 Form
  • Form 2 - Charnwood Community Medical Group New Patient Registration Form

We ask you to complete our new patient registration form at the time of registration. This questionnaire will include questions about lifestyle and medical history and must be completed alongside the NHS registration form in order to register.

If you wish to gain online access to your medical records, we will need to see you in person with some photo ID to verify your identity. This is due to the confidential information accessible on the online access platforms.

Please return completed forms to us by email if you can, alternatively you can hand them in at your local surgery branch.

Registering Online as a Non-English Speaker

You can find guidance and advice on how to become a new patient and the NHS healthcare services you can access, if your first language is not English, or if you're an asylum seeker on the Government website.

Registering Children

Children under 16 need to be registered by their parent or guardian. You can do this by completing the online GP surgery registration form on their behalf. You will need to provide a form of ID to prove that you're their parent or guardian.
